" I've Never Seen a Smiling Face that was not Beautiful "
How much this means to me. Having had my own practice in Bangalore, I have always found all of the patients walkin into my clinic beautiful even behind the pain ! All of them walked in with a concern about their Oral Health even if it was forced by their body. I am here to share my experiences from the time I was allowed to treat patients towards a successful career.. All of us gain information from others' experiences is what I feel. Every new day was a new experience to me and I was getting abundance in knowledge with every passing day ! I quote a saying here by Alfred Shenwold

How can I get Cavities ??

Sitting on my chair and about to start my practice for the day, sipping my coffee and wondering what new I would get to learn this beautiful day .. I heard a patient conversing with my assistant outside. My assistant had a habit of posing a dentist herself with all the teeny bits she hears me do to my patients ! She kind of prepares the patients herself that they are going to be in safe hands with " Doctor amma " ( well, though she is 10 years older to me she insists calling me so !!! ) I have a habit of giving away my best smiles to people with pain. I am kind of convinced with the fact that a smile can ease you a lot of your pain. He smiles back gracefully trying hard to hide his frown , sits down and shoots his question How can i have Cavities , Doctor ?? Like it is a constitutional right not have them . I agree , he shouldnt have them probably because he looked a concerned self. I smiled and said I wonder why too ? Maybe I should be able to explain once you are done, I promised ! I brush my teeth twice daily . I dont have the habit of eating sweets / choclates. I infact hate them. I dont understand how I could have got these cavities, he complained !! Looked like he had given up on advices dentist usually did " BRUSH YOUR TEETH TWICE DAILY " . I put him on the examination chair and did a spree of check up. I started talking .. your teeth are very well indeed except that you had
decay ( cavities ) in two teeth. It is wonderful you brush your teeth twice daily. But can I know how many meals you have in a day ? For which he started explaining his schedule to me.. ( We have to be good listeners you see ) Do you rinse your mouth after every meal ? We need to rinse after every meal to just remove the food substances from our teeth which sit on the biting surface of our back teeth especially. If we look carefully we have fine lines on the upper ( biting / occlusal ) surfaces of our back teeth. They are called the " fissures " . Food substances are more prone to settle in this area and it looks like a path between two mountains. Our mouth already has organisms( from the air we breathe etc., ) which does no mess. But when there are food left behind on tooth.. they have a party. The feast on them and in turn release an acid which breaks down the tooth surface causing cracks called " decay " ( cavities ) . He immiedtly cursed all the bacterias for having had a feast at his cost !! He grunted but wont my brushin in the night help ? I said yes it does . But do you brush regularly. He smiled at me again which meant " Keep this secret between us " !! I coudnt help but smile too. I gave him a disposable brush and asked him to show me how he brushed. Ouch ! I could hear the teeth cursing him now ! I asked him whether he was pretending to show me how hard he brushed.. He said this is how I do.. the more hard you brush the more better isn't ? Says who .. It is not about the how hard you scrape that lovely things that help you bite your food its about how correctly you clean them .
I showed him the correct way of brushing. Yes we have a standard brushing technique which we had to study to clear exams in Dental course. But not every lame human would be able to follow 45 degrees ! Going round over your teeth with a a slight tilt when you reach the gums as if you want to push down the food which is just immdtly below your gums would help. We need to use a soft brush and never go horizantal. As that would wear out your enamel ( The hardest substance in our body ) over a period ! Rinsing / Gargling your mouth after every meal would do a great deal to your teeth. Brushing twice ' regularly' unlike my naughty patient would help yourself too. Massaging your gums with a wet finger will improve the blood circulation in the same. I would also note that some times our fissures are so steep that inspite of religious cleaning a decay could evoke. This is because the brush never cleanses the area no matter what circus we do. Cmon, now you rush to the Dentist !!
How we could help is we take that " drilling machine " as everyone calls it .. ( Carpentors are my brothers !!! ) and widen the steep space. We apply something called a sealant there which covers the area and is usually a flouride content which inturn prevents food lodgement !! By the way .. that is not a drilling machine and it is just a cutting device which helps us remove the decay from your teeth He insisted that is what even he said to drill a hole ! I made no effort to argue but just flashed smile as usual. He said ok doctor I will not use that word in your office again. Good , I said ! So now I had to come to the treatment part and said its nothing but removing the decay with the device and filling it with some good material just like how a leak over a pipe is closed. There I saw him smiling in competition with me and said Is That All ?? Phewww !! Yes that is about it, Mr. Patient !
He got his appointment for the next day and left with a relief that Dentists are after all Plumbers !!!

Dear Preethi,

Very informative.

My younger daughter aged 8 yrs has a problem with brushing....... night times is no no... and morning god alone should help she finishes in 2 min flat. Whatever cajoling, threatening, begging helps. Finally the verdict was out. She is having multiple caries.
Last week one tooth (molar) was extracted and yest two fillings were and done and On Friday two more to go. Doctor has prescribed a new tooth past with fluoride content. My question is how to make these stubborn ones to brush? More than brush .... to do the same properly? Though we have an idea but it would be better if you once again explain to us maybe with small photos. Here the dentist was not helpful in showing her this technique.

Stubborn Kids !

Dear lalitha ,

I am happy to know it helped in a way. What I used to do when I had these lovely kiddos coming to my clinic who seemed careless about brushing was to first show them a decayed tooth picture. I used to tell them they would some day end up with this if they did not brush regularly. But getting the kids to brush twice in a day is quite an effort. The momies have told me.
- You could tell your daughter that if she wanted a beautiful teeth she needs to brush regularly and correctly else her mouth would have a bad odour due to cavities. This worked for me in practice..
- Brushing twice is good. But do not force them to do that, they get all the more adamant then... She will start doing it in course of time. Dont worry.
- Ensure she gargles her mouth after eating toffees and encourage her to brush when she nibbles on sticky food.
- Brushing for 2-5 mins is alrite as far its done correctly. Maybe you should stand by her and tell her if she doesnt do it the right way then maybe you will have to treat her like a baby.
- You also get flavoured toothpastes which will excite the kids.
I wonder why the molar was extracted coz the permament below that comes out between age 10 - 12 . Was it that bad ? This means the space needs to be maintained for the permanent till then. Hope your dentist has explained this to you.
I used to ask my kiddo patients who their best fren was and said he/she would refuse to be frens with u if you had bad breath and black teeth. Do you want that to happen.. You need to just find what could get her to do things the right way except for bribing ( with choclates, dolls etc )

Dear preethi,

You have started an excellent blog.Thankyou very much.

I have some doubts about the root caloning and cavities.
1.If small children get cavities in their milk teeth, is it wise to fill the caviities or leave it like that as anyway it is going to fall.

2.Most people say root caloning treatment does not work for them and even after doing that they have more pain while eating.If root caloning is done, how safe will be the teeth if maintained properly.

3.Even after doing the cavity filling, some people say, they have sensitivity while drinking water.what is the reason for that?.and what is the remedy for that?
Recently my parents were advised by their doctor to brush with colgate sensitive.Is that useful in reducing the sensitivity?

how i am not troubling with so many doubts.

Root Canal Treatment for Primary ( Baby ) Teeth ..

Hi Krithika,

Thanx for your feedback on my space. As I said I love interaction and just here to share all the knowledge I gained from my teachers and experience.

Your questions actually made me start a new blog on Baby Teeth. I would make a detailed description in that maybe by today or tommorow ... Watch my space...

To quickly answer your questions ( like a viva session we used to face during college ! ) ..

- Its is not advised to remove milk teeth unless it is really hard to save and is mobile ( shaking/ wobbling ) . This is because the milk teeth is destined to fall away with the permanent teeth erupting subsequntly at only particular ages by nature. For ex. the front baby teeth ( upper ) falls out between 6-8 yrs and the permanent erupts just behind that. So just in case a kid has his/her upper front teeth removed at age 4 let us say..the permanent teeth will not erupt ( come into) becoz it has been removed.. this would now mean that the empty space needs to be maintained till then. Else there could wrong allignment of the permanent when it does erupt. Meaning it cud be placed either front or back. This would in future require Orthodontic Braces ( Clip treatment ). So a primary teeth can be removed ONLY when there is just less than 6 months for it to shed. Else filling the teeth is the best way.

I do agree when people say that Root Canal hasnt worked for them. I havent usually found any mistake from the Dentist's end except when he/she did not crown ( cap ) the tooth and the filling has been fractured. ( But even then I have seen patients not going back for a crown as it is expensive )
A fractured filling means cracks - way for food - bacteria - decay - infection - failure of Root Canal treatment ( RCT ). If done correctly a RCT is the best treatment to save a tooth these days .
To explain how and when to treat decayed teeth i shall write soon. But be assured if all is done well it ends well

About Sensitivity.. This can arise from any part of the tooth. Either from the filling area ( if it is not done properly or if it has a small crack ) or from the sides of the tooth ( due to enamel loss ). The best treatment is to go back to ur dentist and say u have a problem with the filling. Get a refilling done. For the loss of enamel ther is a temporary ( holds on for 6 months or so) treatment called Bonding where in a liquid under that name is applied over the teeth and treated with light ( Blue ). This acts as a protective layer against cold . Unfotunately Enamel cannot be replaced ( But off late I have seen an gum ad here claiming to induce enamel . I am not sure of the authenticity however ).

The sensitivity in your parents case is because of age. Just like how our muscles wear out on age our enamel too does. This exposes the second layer of our tooth called ' dentin ' which is quite sensitive ( and thats exactly what the enamel did all this while - " protection " ) .Yes using of Desensitising tooth pastes does help over time.

It was an informative blog....you have put down your experience with the patients, laced with humour....
I too have felt about that 'drilling machine' and once while seeing the extraction been done, I felt, Ooph the dentists are supposed to have strong wrist power..ha ha...

I went thru the link regarding how to brush.....Thanx....a couple of yrs back a dentist in India had given us that demostration.....it is another matter that, I sometimes do not follow....old habits die hard no......
And he told us to be very partcular in replacing a tooth within 6 months of extraction...

Well wil get back to you, for advice...

Our lives are humorous isn't ? Just becoz I am a Doctor I dont have to let go the humor at my expense. Every patient has their doubts which may sound silly sometime... But I don't forget that had I not studied Dental i would have had the same kind of doubts. There is no harm in laughing at urself .. atleast you wont have time to care about who else is !!!
I am basically a humorous person and my patients usually leave the consultation room with a deep grin !! And yes ! Our wrists need to be in excellent condition to fight only the mighty molar !!

Hi Dr. Preethi,

I really liked your blog. Very informative and interesting too ! I know I brush way too hard (though more by practice and habit).. My new brush looks so pathetic in just 3 days and I somehow find that more friendly than the newer ones.. I know I should change soon, but I will try. One more thing I find is that, if at all I brush "softly" at times, I am not really satisfied ( looks like there is some psychology too ).. But sometimes if I take sweets or cold stuff, I have a tickling sensation and I just avoid them. Looks like I have "corroded" a good amount of the enamel already.. Also, after coming to the US, I find that my teeth has got an yellowish color and is not white any more, considering I used to have good white teeth when I was in India.. May be the water or again has to do with my not-soft brushing, what ever I don't know.

A very true phrase that a smiling face is always beautiful. Keep it coming more :)

Lets face your finding -
One more thing I find is that, if at all I brush "softly" at times, I am not really satisfied ( looks like there is some psychology too ).. Whoa !!! There is your answer. You are sensitive to sweets and cold bcoz you dont ' brush softly all the time '.. By brushing hard your enamel has been
" abraded " to an extent it seems !!! How else can your poor teeth let you know that you are hurting them ??
Tit for Tat you call this !! Nature is so amazing isn't , Gayu ?
For the unsatisfied feeling you could rub your teeth with wet finger after brushing and that squeak sound will definitly boost you .. Try it ! and have mercy on those poor ones !!! Thanx for writing.. and I love it this way !!

No.. Regular flossing done correctly will not cause gaps between teeth. But thick floss threads does create an invisible seperation.. Actually you get something called an ultra floss from Oral B or even Colgate that is very thin and slides in between even tight contacts. By its make it does not create spacing . Also when flossing is not done correctly .. meanin when it hurts the gums.. the injured gums tend to loosen the teeth over period. So correct way and the right floss is no harm.
